Credit Card Authorization — Read Before You Sign

Custom-manufactured equipment is non-cancellable and non-refundable once production begins. This page exists so nothing about your card, your charge, or your rights is unclear before you authorize us.

Why This Exists

Prestige Hyperbaric (Prestige Hyperbaric and Wellness LLC) takes credit card authorization seriously. Every charge requires a documented paper trail: your ID, your reason, your signed acknowledgment. This is not a phone-verbal transaction.

Section 1 — What You're Authorizing

• You authorize Prestige Hyperbaric to charge your card the amount you specify PLUS a 4% credit card processing fee.
• The 4% fee is charged in addition to (not included in) the approved purchase amount.
• Applicable sales taxes may apply.

Section 2 — The 4% Processing Fee (In Detail)

• Card networks (Visa, Mastercard, Amex, Discover) charge merchants processing fees on every transaction.
• Prestige Hyperbaric passes this fee through at cost — we do not profit from it.
• The fee is 4% of the purchase amount, calculated separately and added to your total.

If you prefer to avoid the fee, you can pay by ACH bank transfer, wire, or check — contact us for instructions.

Section 3 — Custom-Manufactured Equipment Is Non-Cancellable

• Hyperbaric chambers and most manufactured equipment are built to order — production begins only after payment clears.
• Once we place your production order with the manufacturer, the order cannot be cancelled.
• Once production begins, the order is non-refundable.
• This is why we require payment in full in cleared funds before we place any manufacturing order.

Section 4 — Waiver of Dispute & Chargeback Rights

• By signing this authorization, you waive your right to dispute, reverse, or charge back this transaction.
• This waiver applies to the fullest extent permitted by law; fraud protections that card networks are legally required to provide are preserved.
• You confirm the charge is authorized by you, on a card issued to you, and that you have both the intent and the funds to complete the purchase.

Section 5 — Photo ID Requirement

• A clear photo of your government-issued ID is required with every credit card authorization.
• This is our fraud-prevention baseline; no exceptions.
• IDs are stored securely in our Prestige Hyperbaric Google Drive folder, associated with the signed authorization PDF.
• IDs are never sold, shared, or used for any purpose other than transaction verification.

Section 6 — What Information the Form Captures

• Your full legal name
• Billing address (must match the card)
• Email + phone
• Product / model being purchased
• Purchase amount authorized (before the 4% fee)
• Card last 4 digits (we never store full card numbers on our servers — see Section 8)
• Your drawn signature
• Photo of your government-issued ID

Section 7 — What Happens After You Sign

• The form generates a signed PDF and files it directly to our Prestige Hyperbaric records.
• You will receive a confirmation on-screen and by phone/email.
• Prestige Hyperbaric will process the card charge separately using a PCI-compliant processor — the form itself does NOT transmit your full card number.
• You will typically need to provide your full card number verbally by phone at the time of processing, or by a separate PCI-compliant link we send you.

Section 8 — Card Data Security

• This form intentionally does not capture your full card number.
• We use PCI-compliant processors (Square, Stripe, or equivalent) for actual card charging.
• The signed authorization + ID is our permission slip; the processor handles the charge.

Section 9 — Governing Law

• This authorization is governed by the laws of the State of Florida.
• Exclusive jurisdiction and venue is Pasco County, Florida.
• Company: Prestige Hyperbaric (Prestige Hyperbaric and Wellness LLC).
