By signing below you confirm you have read the Site Readiness Review, understand the electrical, delivery, and moving liability terms, and accept responsibility for confirming your site is ready before Prestige Hyperbaric ships your equipment.

Acknowledgments

I confirm the chamber weight (~1,600 lbs on pallet) and that I have arranged appropriate personnel/equipment to move it from the delivery truck.

I have measured my delivery path (doors, hallways, stairs, turns) and confirmed the crated chamber will fit.

I have installed or scheduled dedicated 20-amp 110V circuits per Section 3 of the Site Readiness Review.

I have prepared the placement location, confirmed floor load rating, and allowed the required clearance.

I understand Prestige Hyperbaric's role ends at curbside delivery and I accept moving liability per Section 5.

I have decided on my moving plan (see next question) and accept the associated liability.

I confirm that all artwork, antiques, mirrors, glassware, hallway tables, decor, and other fragile or valuable items will be removed from the delivery path by me or on my behalf before the crew arrives. I understand Prestige Hyperbaric and any referred mover will not touch, move, or handle these items even if I verbally give permission on the day of delivery, and Prestige Hyperbaric is not liable for damage to items that could have been moved out of the path in advance.

I understand that freight, delivery, and installation labor is performed by independent third-party carriers and movers — not Prestige Hyperbaric employees — and that Prestige Hyperbaric does not always know the exact crew, names, or credentials of the individuals dispatched. Any claim for property damage, injury, theft, or crew conduct is made against the company that dispatched the crew and carries insurance, not Prestige Hyperbaric.

I understand that technicians are field labor working under Prestige Hyperbaric’s direction and are not entitled to discuss therapy, product opinions, personal experience, or company matters. I will not ask a technician for their name (except a first name only if required by security to gain entry), phone number, personal contact information, or any off-books work or arrangement outside Prestige Hyperbaric services. All my questions go through Prestige Hyperbaric directly. If I circumvent this process and engage a Prestige-referred technician independently, I assume 100% liability for any damage they cause, and my warranty support is voided on any component they touched.

I have read the full Site Readiness Review and had opportunity to ask questions before signing.
